TRANSPORTATION

This service provides financially challenged patients with a viable way to reach their health care provider.

Transportation services are offered pending volunteer availability; please contact our office at 408-609-3338 as early in advance as possible to make arrangements.

PLEASE NOTE:

· Please make appointments at least five working days before the date of the medical treatment. Transportation service is limited to medical visits after 9:30AM.

· Please prepare a backup plan as we are not always available.
